The link below will take you to approved State Standardized forms that may assist you with the eviction process.
http://www.illinoiscourts.gov/Forms/approved/eviction/eviction.asp. This site also includes updates from the Governor regarding stipulations for evictions.
The Circuit Clerk's Office also has forms available, with less instructions, for attorneys or the public to use. From the Home Page, click on FORMS and then type eviction in the title field and click Apply Filter. The three (3) forms that we offer will appear.
